Aortocoronary bypass surgery in a patient with aortic calcification.

چکیده
A61-year-old woman was referred for aortocoronary bypass surgery. At cardiac catheterization, calcifications of the ascending aorta were noted. To assess the exact extent of aortic calcification, we performed an ECG-gated electronbeam CT (EBCT) study of the thoracic aorta. Our image displays a 3-dimensional volume–rendering reconstruction of the EBCT data set, with thresholds set to display calciumdensity voxels only. The aortic root is completely encased in calcifications, which extend to the aortic arch. The course of the three main coronary artery territories is outlined by calcific plaques. Bypass surgery was successfully completed after cannulation of the ascending aorta at a plaque-free spot in the aortic arch and occlusion of the aorta by a transventricular balloon technique for initiation of extracorporeal circulation. Complete myocardial revascularization was achieved by a left internal mammary artery bypass to the left anterior descending coronary artery and a venous Y-graft from the brachiocephalic artery trunk to the right coronary artery and the obtuse marginal branch.

Mediastinal irradiation has been shown to induce accelerated arteriosclerosis of calcification of thoracic aorta. Some people, with a history of cured malignant diseases, undergo Coronary Artery Bypass Graft (CABG) surgery and the adverse effects of previous radiotherapy on the outcome after CABG is known, but there are few reports on radiotherapy induced coronary and aortic calcification, whic...
متن کاملLeft main coronary artery stenosis after aortic valve replacement.
We describe two cases with iatrogenic left main coronary artery stenosis where a symptomatology of unstable angina appeared within three months of aortic valve replacement surgery. One patient was treated with aortocoronary bypass and the other with angioplasty and stenting.

Aortocoronary bypass surgery in patients with left main coronary artery disease is reported to have an operative mortality of between 1.4 and 39%. It is generally accepted that the operative mortality in this group of patients is considerably greater than in routine bypass candidates, presumably due to the large amount of myocardium threatened by a single lesion. In an effort to preserve threat...
متن کاملImages in cardiovascular medicine. Multiple coronary emboli from a calcified rheumatic aortic valve.
A 51-year-old woman with known rheumatic aortic and mitral valve disease developed progressive exertional angina. Cardiac catheterization demonstrated severe aortic stenosis, mild aortic insufficiency, moderate mitral stenosis, and normal left ventricular function. Cardiac catheterization was not associated with chest pain or ECG changes suggesting coronary embolism during the procedure. Corona...

Seven patients with homozygous familial hypercholesterolaemia, two female and five male, aged 12 to 25 years, underwent clinical and angiographic assessment to define the associated cardiovascular abnormalities. Four patients had angina, two of whom also had syncope on exertion. All had an ejection systolic murmur but no ejection click and a loud aortic second sound. All but one had a systolic ...
